The best in hospice care for seniors.

Hospice Care facilities offer medical care along with emotional and spiritual support for seniors in the final stages of a terminal illness, such as cancer.

In addition to caring for residents, Hospice Care assists families with the practical and emotional challenges of caring for a loved one in the last days of life. Hospice Care may be provided at dedicated hospice centers, nursing homes, hospitals, long-term care facilities, or even in your own home. Wherever you receive it, the purpose is to provide comfort and enhance your quality of life.

Some people may be under the impression that Hospice Care is a last resort, or implies a reduced level of care, which is not the case. In reality, hospice refers to the kind of care that emphasizes quality of life, rather than prolonging life with continued treatment.

Families Loving Their Choice

What a breathe of fresh air knowing that this company provides such a personal touch. They treat their clients with respect and compassion. They take into consideration the details that other companies so easily leave out. It’s a shame more companies aren’t like them, especially working with such fragile situations. They are the best of the best in my eyes. I had the chance to meet the owner and she embodies trust, love and passion for finding the best living/care situation for her clients. Angela is the type of business owner that would literally give you all she has to find what you need. We look forward to a continued relationship with you guys! Cheers!

My wife and I love my mother-in-law but we didn’t have the assisted living expertise to be able to evaluate what’s best for her. Angela (our counselor from LYC) drove out twice and talked to mom. Angela used her terrific interpersonal skills combined with her medical expertise to determine exactly what’s right for mom. Literally dozens of factors were taken into account to find the right place. What impressed me the most is LYC took a comprehensive approach. Both emotional and physical well-being needed to be fully satisfied. Angela showed us a couple places and we eventually found the perfect community for mom. To be brutally honest, it’s not only worked out great for mom but also for us.

Thank heaven we found Living Your Choice!

Recently, our 90-year-old mother became ill and wasn’t going to be able to live on her own anymore, as she’d been doing. My sisters and I were trying to find an assisted living arrangement for her in San Diego, and we were quickly overwhelmed by the options. (It’s very difficult to find a place on your own, just going by the info you see online. Many places don’t post their rates, so you have no idea if you’re looking at something within your budget or not.) I put out a plea on Facebook for any local friends who could recommend a good assisted living place, and a friend suggested we contact Living Your Choice for help.

I wanted to take a moment to thank Living Your Choice for helping me with my dad Bill.

Thank god I had heard of Living Your Choice through a mutual friend. I called and found an expert staff that was able to help me right away. We spent over an hour on the phone discussing my dad, his life, his likes and dislikes, his medical issues, his budget and what he was going to need moving forward. Right away I felt so much better, they understood what was going through and agreed to meet the next morning at the hospital with us both.

Benefits of Assisted Living

Assisted living provides seniors with personalized care, social opportunities, and a safe, supportive environment that enhances independence and quality of life.

Hospice care is specialized end-of-life care focused on comfort and quality of life for individuals with terminal illnesses. It provides comprehensive support to patients and their families, including pain management, emotional support, and spiritual care. The emphasis is on quality rather than curative treatment, ensuring that patients live their remaining days with dignity and comfort.

Hospice care services include symptom management, pain relief, emotional and spiritual support, and assistance with daily activities. The care team, which often consists of doctors, nurses, social workers, and chaplains, works collaboratively to provide compassionate care. These services are offered in various settings such as the patient’s home, dedicated hospice centers, or long-term care facilities.

Eligibility for hospice care is generally determined by a physician and is intended for patients with a terminal illness and a prognosis of six months or less if the disease follows its normal course. The focus is on comfort and quality of life rather than curative treatments. This care model supports both the patient and their family during a challenging time.

Hospice care extends support to family members by providing counseling, respite services, and bereavement care. The care team works closely with families to help them understand the disease process and manage the emotional challenges associated with end-of-life care. This holistic support system helps families cope with grief and prepares them for the loss of their loved one.

Pain and symptom management in hospice care are managed through personalized care plans that prioritize comfort and quality of life. Medications, therapies, and non-pharmacological interventions are used to alleviate pain and other distressing symptoms. Regular assessments and adjustments ensure that the patient’s comfort remains the primary focus of care throughout the end-of-life journey.

The interdisciplinary team in hospice care includes medical professionals, nurses, social workers, spiritual counselors, and volunteers who collaborate to provide comprehensive support. Each member contributes their expertise to address physical, emotional, and spiritual needs. This coordinated approach ensures that the patient and family receive well-rounded care that honors dignity and provides comfort during a critical time.

Yes, hospice care can be provided at home, allowing patients to remain in a familiar and comforting environment. Home-based hospice services include regular visits by the care team, medication management, and supportive counseling. This option enables patients and their families to receive compassionate care while maintaining a sense of normalcy and comfort in their own surroundings.

It may be time to consider hospice care when a patient’s illness is no longer responsive to curative treatments, and the focus shifts toward comfort and quality of life. Discussions with healthcare providers, family members, and the patient can help determine the appropriate time. Hospice care is designed to support those in the final stages of a terminal illness, ensuring compassionate care for both the patient and their loved ones.

Hospice care offers extensive emotional support through counseling, support groups, and spiritual care services. The interdisciplinary team is dedicated to addressing the emotional and psychological challenges faced by both patients and families during this difficult time. These supportive services aim to ease anxiety, provide comfort, and help individuals process their feelings while facing the end-of-life journey.

To access hospice care, you should speak with your healthcare provider about your loved one’s condition and explore hospice eligibility. Once eligibility is confirmed, a hospice team will conduct an assessment and work with you to develop a personalized care plan. This process ensures that the transition to hospice care is smooth, compassionate, and centered on preserving dignity and comfort.
